Once you have complete the forms, you may either call our office to schedule an appointment, or someone from our office will follow-up with you to find a time that works best with your schedule.

We kindly request you arrive 10 minutes prior to your appointment time to complete your registration with our front office staff. Please come prepared with your insurance card, photo ID and comfortable clothing that will allow your provider to perform the necessary evaluation – tank top or t-shirt for patients with upper extremity concerns, shorts or loose fitting pants for patients with lower extremity or back issues.
We have reserved your appointment time for you in order to provide individualized attention. We respectfully require 24 hours notice if you are unable to make your chosen time. We will do our best to offer a more suitable time, as we are able. Those not giving 24 hours notice will be charged a cancellation fee.
